Tricky Medical Situation

I can not believe that some 13 years after I had a medical bill gone unpaid by my insurance company that it is somehow turning up on my credit report. Just the other day I received a phone call from a very rude creditor saying that I owed $87 for an emergency procedure. My insurance was suppose to have paid this bill and I remember the incident quite well. How can I get a debt validation from this creditor? I want to find out how, after so many years, that this can appear once again on my credit report when it should have already been cleared.
